At this time, additional insured requests must be made directly to your agent. Contact your agent by phone or email to add additional insureds.

Be sure to have the following items ready when making your request:

1. Named Insured – The person making the request to endorse the policy must be a named insured or a member or managing or the named insured corporation.
2. Project Start Date
3. Approximate Project Completion Date
4. Description of the Scope of the Project
5. Additional Insured’s Information
